We have co-developed this guide with a lot of stakeholder involvement and engagement activities with patients, carers, members of the public, GPs and pharmacists. The guide aims to support patients and carers to be more actively involved in their patient safety in primary care which is a key national and international priority for patient safety and care. It is also available as a paper booklet, and a website (https://www.patientsafetyguide.org/). The Guide app is free to download and use. It does not provide medical advice, diagnosis or treatment.
